                        <title>Intermountain Health ACO Ranked Among Nation’s Top Five for Medicare Savings in 2024</title>

                        <guid>https://news.intermountainhealth.org/intermountain-health-aco-ranked-among-nations-top-five-for-medicare-savings-in-2024/</guid><pp:caseid>815654</pp:caseid><description><![CDATA[<img src="https://content.presspage.com/uploads/2804/f1b8ddee-bcc5-47eb-be23-ff4584264909/1920_adobestock-417122027.jpeg?10000"><p><span>Intermountain Health’s accountable care organization (ACO) generated more than $67.6 million in Medicare savings through the Accountable Care Organizations Realizing Equity, Access, and Community Health, or ACO REACH, program in 2024.</span></p><p><span>The results placed the ACO fifth nationally among participating organizations and marked the third consecutive year Intermountain ranked among the nation’s top five for gross savings. The ACO also earned a 100% quality score for the third consecutive year, qualifying as a High Performer, with nationally leading results on the ACO REACH hospital readmission measure.</span></p><p><span>The performance reflects Intermountain’s continued leadership in proactive, coordinated care — work now advanced through Proactive Care Support Services, or PCSS. PCSS brings together teams and capabilities focused on identifying patient needs earlier, improving access to care, coordinating support across care teams, and helping patients stay healthier at home.</span></p><p><span>“Healthcare achieves its mission when we invest in keeping people well. These results reflect Intermountain's shift to proactive care with focused collaboration between physicians, care teams, and community partners to identify needs early, coordinate care more effectively, and help patients stay healthier at home. That's the future of healthcare, and we're proud to be leading that work," said Dr. Steve Clark, chief medical officer for PCSS at Intermountain Health.</span></p><p><span>The impact also extends to primary care practices. In 2024, Intermountain’s ACO returned $31 million to primary care clinic providers, helping practices invest in services that matter to patients, including improved access, stronger follow-up support, chronic condition management, and health initiatives for underserved communities.</span></p><p><span>“The sustained improvement in quality, patient experience, and cost performance reflects Intermountain’s commitment to partnering with leading primary care providers and health systems to deliver best-in-class care for patients throughout the Mountain West,” said Justin Brooks the administrator of the ACO.</span></p><p><span><strong>What accountable care efforts mean for patients</strong></span></p><ul style="list-style-type:disc;"><li><span><strong>Fewer avoidable hospital visits:</strong> The ACO achieved nationally leading performance on the ACO REACH readmission measure and continued top-tier performance in reducing unplanned hospital admissions.</span></li><li><span><strong>Better communication:</strong> Patient experience improved based on Consumer Assessment of Healthcare Providers and Systems measures, including how well providers communicate with patients.</span></li><li><span><strong>More appropriate care:</strong> Better use of healthcare resources means patients are less likely to receive duplicative or unnecessary services.</span></li><li><span><strong>More support between visits:</strong> Proactive outreach, care coordination, and chronic condition management help patients get support earlier and stay healthier at home.</span></li></ul><p><span>Together, these results show how proactive care can improve quality, strengthen the patient experience, reduce avoidable hospital use, and lower costs.
