Submit medical and religious exemptions for the COVID-19 vaccine by December 1

Consistent with Intermountain's practice for other required vaccines, you can apply for a medical or religious exemption from the COVID-19 vaccine requirement. Exemption requests must be completed and submitted by Wednesday, December 1.

• A medical exemption is defined as identified contraindications and precautions for each vaccine from the Centers for Disease Control and Prevention (CDC). Medical documentation from a provider (MD, DO, or APP), as outlined in the Intermountain medical exemption form for an identified condition, will qualify as a medical exemption.
• A religious exemption is a sincerely held personal religious belief and/or a religious tenet that prevents a caregiver from receiving a vaccination. Political and/or philosophical beliefs don't qualify for a religious exemption. Documentation from a religious leader isn't required.

All requests are reviewed weekly by diverse panels of Intermountain professionals, and caregivers are notified about the committee's decision by email. If you're approved for an exemption, you'll be required to mask. There may be a COVID-19 testing requirement for those with an approved exemption, per federal regulations or for other safety reasons, and you'll be notified if/when that guidance changes.
